NPWT Case Sharing for Proximal Fracture of Right Tibia and Fibula

A 57 year old male suffered a fracture of the proximal end of the right tibia and fibula due to work-related injury, and the front bone of the right tibia exposed

Negative pressure wound therapy (NPWT) is a method of extracting fluid and infection from wounds to help them heal. Seal the wound with a special dressing (bandage) and connect a mild vacuum pump.

Doctors may recommend NPWT for burns, pressure ulcers, diabetic ulcers, and chronic (long term) wounds or injuries. This treatment can help patients heal faster and reduce infection.

Departments used in this series and corresponding indications:

Trauma orthopedics:

Bone exposure combined with infection, steel plate exposure combined with infection, tendon exposure combined with infection, infection after limb external fixation, limb soft tissue defect and necrosis; Avulsion injury with infection, open fracture with soft tissue defect, open long-term non healing wound, protection of skin graft area before and after skin grafting, osteomyelitis, sinus and osteofascial compartment syndrome

Burn department:

Shallow second degree burn / deep second degree burn, back of hand thermal crush injury, fresh burn wound treatment, old burn wound treatment, abscess after perineal burn, TBSA 5% burn

Severe back burn, firearm impact injury, explosion injury

Chronic wound:

Hand foot chronic ulcer, diabetic foot ulcer nonunion wound,

Chronic ulcer of limbs, sacrococcygeal ulcer, bedsore ulcer

Emergency Department:

Avulsion injury, degloving injury, destruction injury, soft tissue defect and bone tissue exposure

Soft tissue defect cannot close in one stage and wound repair after amputation

Hand and foot microsurgery:

Severed lower limbs, hands and arms

General surgery and cardiothoracic surgery:

After radical mastectomy, wound repair, radical resection of rectal cancer, refractory incision, stoma, chronic empyema, esophagus anastomosis, pleural fistula, stoma fistula, etc.

Pu sponge in the picture

Pu sponge is a dry sponge, and polyurethane material is the best thermal insulation material in the world. Known as the "fifth largest plastic", it can different physical properties such as density, elasticity and rigidity by modifying the formula; Application in wound attachment; It has advantages in managing exudate, which is manifest in high drainage capacity, especially suitable for severe exudate and infected wounds, promoting granulation tissue formation and ensuring uniform transmission pressure.

Tips: sticking negative pressure sponge will encounter various problems. For example, patients with low albumin should suspend negative pressure, supplement protein first, and then do negative pressure after stabilization, otherwise there will be too much protein loss, which is prone to the risk of shock.
